Below is a guide to help you complete your application.

Please follow the guide below to help us process your application.

1. Complete the application form using the guidance below. It is important that you complete the form as fully as possible to avoid delays in processing your application.

2. Ensure you sign and date the form.

3. If you will be under 18 at the start of your course, please ensure that your parent/guardian sign and date the form.

4. Hand the reference to your current/previous school or college for completion.

5. Hand in the completed application form to your current school as soon as possible (some schools will ask you to send it directly to the college)

Section 1 Personal Details

• Please complete this section carefully. If you change your current address or telephone number, please let us know so we can amend our records.

• Ensure you let us know if you have been resident
in the UK/EU for the past 3 years, if not, please
let us know where you have lived.

Section 2 School/College or Employer

• Let us know which school or college you are currently or were previously attending.
• If you are over 19, please fill in details of your employer details.

Section 3 Course Choices/Campus

• If you are applying to more than one campus, please indicate your order of preference. This information is for statistical purposes only
• You do not have to fill in all boxes!

Courses at Lewes

If you are applying for a mixed programme at Lewes, you will NOT be interviewed for each subject.Normally applicants will have one interview during which their whole programme of study will be discussed. If you are unsure about one of your subject choices, put this one first: we can then try to arrange for you be interviewed by a teacher
from that subject area, so you can receive more information. Please note that some courses require you to attend an audition and/or assessment as part of the interview process.

Courses at EVoC and Park

If you are applying for a Vocational course at EVoC and a GCSE or A Level programme at Park, you WILL have two interviews.

Section 4 Disability/Learning Difficulty

• We are committed to meeting the needs of people who are disabled or who have learning difficulties. Please let us know if you need additional support by ticking the box and we will send you a Support Needs Questionnaire for your completion and return.

Section 5 Ethnic Origin

• This information is required by the Learning and Skills Council (LSC) for monitoring purposes.

• These sections will enable us to ensure that programme you are considering meets your career aims and will help us to assess your application.

Section 7 Qualifications

• Please complete this section as fully as possible. Make sure you include all the examination subjects you are currently taking, together with the level (e.g. higher, intermediate or foundation).
• Please include your predicted grades and if you have already taken any exams, include the grade achieved.

Section 8 Marketing and Contact by LSC

• Tick the boxes if you wish to be excluded from mailings or contact.

• Please read this section carefully.

Section 10 Signature

• Ensure you sign and date the application form.
• If you will be under 18 at the start of your course, please also ensure that your parent / guardian sign the application form as well.
